The following describes the fields on this screen that may not be self-evident. Site Code, Specialty Code, Medicaid #, Blue Shield #, & Other ID # are relevant only if you submit insurance claims electronically.
If you use RECS or Med E America to process your electronic insurance claims, enter ECF.
If you use Envoy, enter 1111.
Code representing the doctor's specialty.
Code Speciality 301 General Practice 302 Independent Hygienist 303 Endodontics 304 Pedodontics 305 Periodontics 306 Prosthodontics 307 Orthodontics 308 Denturist 019 Oral Surgeon
If 'Licenses' in the 'Options Purchased' box is less than 2, enter the number assigned by the insurance carrier to represent the doctor. Otherwise, enter the number assigned by the insurance carrier to represent the practice, and enter the numbers assigned by the carrier to represent the individual doctors on their individual practitioner records. See Edit Practitioners.
'Other ID #' is used for any carrier that has assigned the doctor an ID number other than Medicaid or Blue Shield (typically Delta Dental).
This setting is not relevant if 'Licenses' in the 'Options Purchased' box is greater than 1. If you would like your name to appear on insurance claims as it is displayed on the 'Company Name' line on this screen, click in the circle to the right of this option. If you would like it to be your First Name, Middle Initial, Last Name, and Degree as they are displayed on this screen, type click in the circle to the right of this option.
